Helping employees navigate through all seasons of life
Lytle EAP Partners’ medical advocacy program, Personal Health Partners, is a new approach to maneuvering through the healthcare system. It offers strategies to promote employee health, productivity, and well-being by serving patient populations throughout the entire lifespan and by addressing health problems in every category of disease classification and in all disease stages.
Have you ever experienced any of these situations?
Been on the phone for hours with an insurance company, transferred multiple times, but all you need is a simple answer?
Had a young son or daughter who needs special medical attention, but you’re being told there’s a four-week wait for an appointment?
Had an elderly parent discharged from the hospital, but is unable to stay at home alone and now needs a place to live?
Our professional Personal Health Partners staff can resolve problems and provide a steady hand when employees are most challenged by life events. Employees will have access to licensed social workers, registered nurses, and certified case managers. Members of our network are experts in talking with health insurance representatives, doctors’ offices, claims representatives, health care experts, and community resources.
Our Personal Health Partners program provides assistance with:
- Advocacy and Research
- Discharge Planning
- Elderly and Aging Issues
- Care Transition
- Medical Appointment Preparation
- Healthcare Transportation
- Durable Medical Equipment Coordination
